Terms of Use and Data Processing
1. Purpose
Careforms is a platform for sending clinical forms and following up patients by the contracting clinic or health professional.
2. Roles under LGPD
The clinic is the Controller of its patients' data and Careforms acts as Processor, handling data only under the clinic's instructions and to provide the service (LGPD, arts. 5, 37 to 40).
3. Sensitive health data
Form answers are sensitive personal data (art. 11). The clinic ensures the proper legal basis, such as health protection or patient consent, collected by the platform before each answer.
4. Confidentiality and access
Users keep professional confidentiality and access only patients under their care. Every access to clinical data is recorded in an immutable audit trail. Careforms staff do not access clinical answers.
5. Security and retention
Data is encrypted in transit and at rest. Records and answers are kept for at least 20 years (CFM Resolution 1,821/2007), including in case of non-payment, when access becomes read-only.
6. User responsibilities
Users are responsible for keeping their password safe, for the accuracy of registered data and for using the platform according to their professional ethics and applicable law.
