A post-operative questionnaire the patient answers from home
In the days after surgery, Careforms asks the patient how they are doing, every day or as often as you set. The answers arrive organized, with charts and alerts, so the team can act early.
What to ask
A good post-operative questionnaire is short: pain from 0 to 10, fever (yes or no), bleeding, how the dressing looks, medication use, nausea, sleep and room for questions. In Careforms you build these questions in minutes or ask the AI assistant (depending on the plan) for a draft to review.
Date questions record, for example, when a symptom started.
Scheduled daily sends
Schedule the send at the time you choose. By email it goes out on its own; on WhatsApp the message is ready for the team to send with one tap. If the patient did not answer the previous send, the new one replaces it, and late answers are plotted on the right date.
Alerts and progress
Set alert limits (depending on the plan): pain above the value you choose or fever checked is highlighted in notifications. The progress chart shows whether the recovery is on track, and the report can be saved as a PDF.

Does the patient need an app?

How long is the link valid?
Each questionnaire link is valid for 14 days. With recurring sends, each new send brings a new link.
