Why measure what the patient reports

Much of what matters in a treatment only the patient knows: how much it hurts, how well they sleep, what they can do day to day. Measuring it regularly shows whether the treatment works and helps you decide together with the patient.

On paper, PROMs get lost and rarely become a chart. In Careforms, each answer goes into the patient's series automatically.

From questionnaire to chart

Build the questionnaire with scales, choices, yes or no, dates and text, or adapt the standard treatment follow-up questionnaire. Choose how each question appears in the chart (line, bars, pie or timeline) and schedule the sends. When you edit a question, old answers stay in the same series.

Besides each patient's progress, the dashboard combines the answers to a questionnaire for your patients or the whole clinic (depending on the plan), with adherence and how many are improving, stable or getting worse.

Frequently asked questions

Can I use validated scales, such as pain or anxiety scales?

You can build the questions of the scales you already use in the builder. Check the license of each instrument: Careforms does not ship third-party scales.

Do patients actually answer?

Answering is simple: a link on WhatsApp or email, a 4-digit code, large screens and nothing to install. The dashboard shows each questionnaire's adherence so you can see who is not answering.

Can I export the data?

The responsible professional can export the patient's data and save the progress report as a PDF (depending on the plan). Every access to patient data is recorded in the audit log.
