1. Who processes your data
The clinic or health professional who sent the form is the Controller of your data. Careforms is the Processor platform, storing and processing the information only for that purpose.
2. Which data and why
Name, contact, date of birth and form answers, used exclusively for your health follow-up by the clinic, based on health protection and your consent (LGPD, art. 11).
3. Who can see it
Only the professional responsible for your follow-up sees your answers. Every access is recorded. Careforms staff do not access clinical answers and your data is never sold or shared for advertising.
4. Security and retention
Data is encrypted and kept for the period required for health records (at least 20 years, CFM Resolution 1,821/2007).
5. Your rights
You may request access, correction or a copy of your data directly from the clinic, or write to contact@nexagil.com (LGPD, art. 18).
6. Website cookies
On the public pages of the website (home, plans, sign-up and articles) we use the Meta Pixel to measure our ads. It is not loaded in the clinic area or in patient forms, and no health data is sent to Meta. You can block it in your browser settings or in your Facebook and Instagram ad preferences.
