What is Careforms?
Careforms is a Brazilian platform for following up with patients between visits. The health professional sends questionnaires via WhatsApp or email, the patient answers on the phone with a 4-digit code and the answers become treatment progress charts.
Who is Careforms for?
For health professionals in any field (physicians, dentists, psychologists, physiotherapists, nutritionists, speech therapists, nurses and others) who work alone or in clinics.
Does the patient need to download an app or create a password?
No. They open the link they received, type a 4-digit code and answer in the phone's browser.
What kinds of questions can I use?
Short and long text, scale, single choice, multiple choice, yes or no and date (with time, if you want). You can start from the standard questionnaire, build your own or ask the AI assistant for a draft (depending on the plan).
How do I see whether the treatment is working?
On each patient's page, the answers appear in charts over time, showing whether each measure is improving, stable or getting worse. The report can be saved as a PDF (depending on the plan).
Is there a free trial?
Yes, with no card. At the end of the trial you choose a plan and pay by Pix or card; if you do not subscribe, the account becomes read-only and nothing is deleted.
How much does it cost?
Plans are monthly, with discounts on longer cycles, and differ by features and number of health professionals. There is no free plan. Prices are on the pricing page.
Does Careforms issue invoices?
Yes. Every payment generates the service invoice automatically, sent to the billing contact.
Does Careforms comply with the LGPD?
Yes. The patient consents to the processing of health data before answering, each professional sees only their own patients, every access is recorded and the Careforms team does not read clinical answers.
Does Careforms replace the electronic health record?
No. It handles follow-up between visits and works alongside the record you already use.
