What happens between sessions matters
Mood, anxiety, sleep and difficult moments change from one day to the next, and on the day of the session people tend to remember only the last few days. Regular questionnaires record that path, with dates.
You build questions with scales (0 to 10, for example), choices, yes or no, dates and free text, and set how often they are sent. Answers stay organized per patient, ready for the conversation in the session.
Easy for the patient to answer
The patient gets a link by email or WhatsApp, types a 4-digit code and answers on the phone. No app to download, no account and no password to remember. If you publish the questionnaire in English or Spanish, they can also answer in their own language.
Professional confidentiality first
Each professional sees only their own patients and only the answers to questionnaires sent in their name. Even in a clinic with several professionals, nobody else reads the answers. Every access to patient data is recorded in an audit trail, and the Careforms team does not access clinical answers.

Does Careforms replace the clinical record?
No. Careforms handles follow-up between sessions: sending questionnaires, answers and progress charts. Session notes stay in your clinical record.
Can I use my own questions and scales?
Yes. Start from the standard follow-up questionnaire or build your own in the form builder (depending on the plan), with scales, choices, yes or no, dates and text. An AI assistant can also draft one from your description, and you review it before publishing.
Do I need to enter my council registration (CRP) to use it?
Not to create the account. Signup asks for name, email, WhatsApp, password and CPF or CNPJ. Profession and council registration (CRP) are added later, in Team, and the registration is optional.
