Know whether the treatment is working before the return visit
Between visits, Careforms asks the patient about symptoms, medication side effects and quality of life, and shows the answers in progress charts. You arrive at the return visit with weeks of data, not just that day's.
Symptoms and scales in charts
Each scale question becomes a time series. Careforms shows whether each measure is improving, stable or getting worse and lets you compare two questions on the same chart, such as pain and sleep. Late answers are plotted on the date they refer to.
Risk alerts
Set limits on scales or mark answer options as alerts. When a patient answers something worrying, the answer is highlighted in notifications (depending on the plan), so you can act before the next visit.
For multi-specialty clinics
A patient can be followed by several professionals at the clinic, and each one sees only the answers to questionnaires sent in their name. When a professional leaves, they hand their patients over to a colleague, with history and scheduled sends.

Frequently asked questions
Is Careforms an electronic health record?
No. Careforms is a platform for follow-up between visits (questionnaires, answers and progress) and works alongside the record you already use.
Is the data secure?
Yes. Data travels encrypted (HTTPS) and is kept on AWS servers. The patient consents to the processing of health data before answering, every access is recorded and the Careforms team does not read clinical answers.
Can I send questionnaires in another language?
Yes (depending on the plan). A questionnaire can have Portuguese, English and Spanish versions; the patient answers in their language and you read in yours.
